I agree to be virtually evaluated by the doctor physically located elsewhere.
I understand that the process of teleconsultation depends on the availability of adequate bandwidth and occasionally there could be disruptions in the quality of video and audio.
I understand that there could be a delay in the teleconsultation due to unavoidable reasons.
I understand that providing a truthful medical history, past and present is absolutely essential to help the Teleconsultant to arrive at a differential diagnosis.
I understand that to establish a diagnosis and suggest appropriate management, on occasions further investigations may be required.
I understand that on occasions the investigations already available with me, made available to the tele consultant maybe deemed to be inadequate or unreliable by the Teleconsultant.
I understand that in some instances there could be some limitations of a teleconsultation and a face to face direct consultation may also be necessary.
I understand that in addition to real time audio video consultation, the Teleconsultant may also review the medical history offline through email/chat portal.
I understand that in an emergency, the Teleconsultant may advise the patient and his/her caregivers that the patient has to be taken to the nearest hospital capable of handling the situation.
I understand that the information provided by the patient may be used for analysis research, training & even disclosure if required in keeping with the prevailing rules & regulations.

*Addition ,Alteration or Cancellation of Tests:

Tests can not be cancelled or altered once the sample is taken for examination.
If the sample has not been taken for examination ,tests can be altered or added by providing written request to Medicohelpline through email and follow - up over telephone. The email ID for this purpose is accession admin@medicohelpline.com Please mention appropriate details like Prefred Partnere center name , test addition or alteration ,patients name and requester identity is such an email.
Additional tests will be done till sample stability conditions are met.

Clinicopathological Correlation

Clinicopathological correlation is very important for a positive on patient care.
Please cooperate & provide history along with the samples for all patients & tests .
Clinical symptoms & significant radiological findings should be sent .
Please fill the necessary details in the TRF.
